Forms Required for OJI Referee Testing

The following is required to test:

1)  OJI Officials Certification Application  (Click HERE for a blank copy)

2)  A copy of your last referee certificate (if you had previously been certified)

3)  A copy of your current OJI/USA Judo registration card

4)  A list of events worked since your last referee certification  (include date, event name, city and state where event was held)
