Mat Rental - December 12th, 2009

The Ohio Judo Inc. Board of Director passed this procedure for renting mats from Ohio Judo Inc. until further notice.

1. Mats must be transported by the renter
2. They must use their own Truck & Trailer to haul the mats in
3. They must unload and load the mats back up into the OJI Trailer when renting the mats at the storage building, OJI Trailer MUST remain at the Storage Building.
4. Mats must be RETURNED undamaged and on a timely manner, if mats are damage the renter is responsible to replace the mats
5. Deposit must be made when reserving the mats
6. Rent fees must be paid in full before picking up the mats
7. Only DUCT TAPE is to be used on the Mat/Mat Cover, You must provide your own Duct Tape

This Rent Agreement Includes the Mats and Mat Covers ONLY
Not included: Duct Tape ---- Blue/White Belts ----- Score Boards ----- Flags/Stop Watches ------ PA system

Rental Fees:
Ohio Judo Inc. Clubs (Must hold a Current Club Charter with Ohio Judo) Includes 2 Days
1 Mat/Mat Cover – $15.00
2 Mats/ Covers – $30.00
3 Mats/Covers - $45.00

All Other Clubs Includes 2 Days
1 Mat/ Cover - $75.00
2 Mats/Covers - $120.00
3 Mats/ Covers – $150.00

$100.00 Deposit is require when reserving the mats Deposit will be return when Mats are returned and check in by OJI

Ohio Judo. Inc. Mats/ Covers Rental Agreement

Ohio Judo, Inc. (“Lessor”) herby rents to ___________________________________________(Name), (“Renter”) the mats and mat covers described on the Mat Rental Procedure Form. Renter in turn agrees to comply fully with the terms and conditions of this Agreement, as specified further herein. The parties acknowledge that the Mats/Mat Covers rented hereunder is intended for Renter’s use at ______________________________________________________ (Name of Event)

1.Renter acknowledges and agrees that the Mats/Mat Covers is in good condition and is appropriate for Renter’s intended use, unless the Renter notifies Lessor to the contrary within one (1) hour after(Laying down the Mats) the rental period. Renter agrees to return all Mats/Mat Covers to Lessor in the same condition as when received by Renter, expect for normal wear and tear, at the OJI’s Mat Storage Center before or no later than 5PM (Day after the Event) on the due date specified herein.

2. RENTER FURTHER EXPRESSLY AGREES TO ASSUME FULL AND COMPLETE RESPONSIBILITY AND LIABILITY FOR ANY ALL DAMAGES TO THE MATS/COVERS AND LOSS OF USE OF THE MATS/COVERS WHICH OCCURS WHILE SUCH MAT/COVERS IS IN USE BY OR IN THE POSSESSION OF THE RENTER, INCLUDING BUT NOT LIMITED TO REPLACEMENT AND REPAIR COSTS, AND TO HOLD LESSOR HARMLESS IN CONNECTION WITH ANY SUCH DAMAGES AND LOSS OF USE, INCLUDING BUT NOT LIMITED TO REPLACEMENT AND REPAIR COSTS.

3. Renter agrees that in the event of loss of damage to the Mats/Covers while such Mats/Covers in its possession pursuant to this Agreement, whether or not Renter causes such loss or damage. Renter shall be responsible for and shall pay to Lessor (on demand) the amount of loss or damage as applicable, including but not limited to the loss of use of the Mats/Covers expect as otherwise provided by law. The cost of the loss of use of the Mats/Covers will be based on the replacement cost of the Mats/Covers.
