1. Club Registration  (This section contains 2 forms)

Download these 2 forms:

Please write 2 checks:

1.  1st check  for $50 to USA Judo
2.  2nd check for $25 to Ohio Judo. 

Mail the Club Application form and both checks to:

Form #2.  USA Judo Background Application.doc

The Background Application has a separate fee and should be mailed (with a check for the fee) to the address on the form, NOT to Ohio Judo.

2. Sanctions (This section contains 3 forms)

Before your event: (1 form)

Form #1:  Download this form:  Application_for_Sanction_2008.pdf.  Fill out the form.  Please include 1 check to USA Judo based on the # of expected competitors and include a 2nd check to Ohio Judo for $25.

Please mail your Application for Sanction and 2 checks (Check #1 to USA Judo and Check #2 to Ohio Judo) to:

Please download both forms:

The Tournament Director should complete both forms and mail them USA Judo, NOT to Ohio Judo.

Membership in Ohio Judo Inc. provides the following advantages:

For USA Judo Members:

1.        Protection of amateur standing in all tournaments that USA Judo sanctions in the United States.

2.        Ability to participate in any USA Judo-sanctioned events in the U.S. for which he/she is qualified. This includes clinics and training camps. Furthermore, all National tournaments sanctioned by either USJF or USJA are open to qualified USA Judo members.

3.        Opportunity to represent the USA at the Pan American Games, Pan American Championships, World Championships, select international competitions and the Olympic Games!

4.        Participation in International Judo Exchange Programs.

6.        One million dollar liability insurance coaches or officials at sanctioned events or scheduled training sessions. No deductible.

7.        Excess Accident Medical Insurance for any injury occurring at a sanctioned event or scheduled training session. Membership includes a $25,000 medical insurance policy with a $2,500 deductible. "Excess" coverage means that it goes into effect after any other health insurance you might have.
